For an initial treatment evaluation:
Dr. Martone offers a 10-minute initial screening phone call to determine if he may meet your needs. At that point, an initial intake appointment will be scheduled with the potential for 1 additional intake visit, depending on the complexity of your case.
Dr. Martone’s private practice is fee-for-service. He does not accept private insurance and payment is due in full at each treatment session. He can provide billing documentation to assist you with filing for out-of-network reimbursement from your insurance provider. Insurance that includes out-of-network benefits will often reimburse anywhere between 20 to 90% of fees.
In reviewing coverage with your insurance carrier, it may help to provide the codes that may be billed. You will need to check if you have out-of-network benefits, the deductible for out-of-network benefits, and the dollar reimbursement amount for the following codes:
Initial Evaluation: 90792
Medication Management Appointment: 99214 or 99213 (depending on complexity)
Medication Management with Therapy Appointment: 90833 or 90836
For the first appointment cancelled less than 48 hours in advance, there is a one-time waive of the no show fee. For any subsequent cancellations, payment for the missed session is due in full.

Dr. Martone offers an initial screening phone call to determine the scope of the evaluation required for your case. Records must be received before the evaluation can take place.
